NTSB Safety Recommendations Following San Bruno Incident

NTSB has issued seven safety recommendations following the September, 2010 San Bruno incident in California.

#1. To PHMSA: Inform pipeline operators about circumstances leading up-to and consequences of pipeline rupture at San Bruno and NTSB’s recommendations.

#2. To PG&E: Search records to identify all the gas transmission lines (in class 3 & 4 locations, and class 1 & 2 high consequence areas) that do not have MAOP established through hydro-testing.

#3. To PG&E: Determine the MAOP for pipelines found through #2. This MAOP to be based on the weakest section of pipeline.

#4. To PG&E: For pipelines, where MAOP cannot be determined, use a spike test to determine MAOP.

#5. To CPUC: Oversee PG&E’s implementation of #2

#6. To CPUC: Oversee PG&E’s implementation of #4

#7. To CPUC: Inform California interstate natural gas operators of NTSB findings and recommendations (same as #1)
