Does this explain why Hazards are missed and Risks go unnoticed?

  1. While the narrow focus or the failure to see the forest for the trees error certainly can be a cause, my experience is that it is related more to four causes. The first is best described by a term that was attributed to astronaut Frank Borman speaking at the Apollo 1 investigation hearings – “A failure of imagination,” secondly to a hubris – “That can’t happen here,” thirdly, to a lack of competency and/or experience (throw in ignorance here to), or fourthly – to dismissal by management.

  2. I somewhat question what this video really shows. The gorilla is visually not far disimilar to the people wearing black clothes and viewers are focused on the white.

    Also, taking into account the initial aim of the video (to count the number of passes), the gorilla, while funny, is irrelevant.

    I think its hard to compare noticing an irrelevant detail with noticing something in a dangerous enviroment.
